[Plugin: Contact Form 7] Appointment Calendar

Trying to hunt around, and I’d love an appointment calendar if at all possible, if anyone knows where there is one, that would be appreciated also. I need it to work with this plugin, and have it so if i were to make a form, and add in that little spot, they could choose a date on a calendar, and that would show up in the mail as “mm/dd/yyyy” or something along those lines, or maybe even like “November 24th, 2008” or “Monday, November 24th, 2008”
