[Plugin: Contact Form 7] Any tag creates same result

The only result I can create is a single text box. At the bottom, next to the submit button, it states, “Text only. No markup allowed.”

I have even removed all tags, get the same result. I have used the defaults tags, same result. Created multiple tags using the generator, same result. Copied code from the quick start guide, same result.

Is there a conflict with the Avisio theme?
