 * Greetings — I like this plugin a lot so far, but a little bit of instruction 
   on its use would go a really long way toward making it more usable.
 * Specifically, I need to know:
 * Where is the “filter hook for template” located, and/or where should this code
   be placed in my page template?
 * Where is the actual template file?
 * Where is the “filter hook for stylesheet” located, and/or where should this code
   be placed in my page template?
 * If the “Default source of template” is not located in a separate file, where 
   is it located?
 * How to create a brand new template, and/or edit the existing template?
 * Any help would be appreciated. Thanks!
