I’ve not been able to reproduce this on 2.8.5 (which this should work on just fine).
I have @bk.ru (for example) blocked and it seems to work just fine.
Can you give me an example of the domains?

Here’s an example list that I am still getting sign-ups with even though they are in the Ban Hammer blacklisted domains list:

not sure why this isn’t working for me… BTW, I have upgraded WP to 2.8.6
Try putting the @ in front.

What are your server specs? PHP version, SQL version, etc.

We’re running the latest WordPress and BH went on yesterday. It’s still being ineffectual.

Sadly, no. I’m really just stumped as to why a server with Zend and eAccelerator break, but APC is happy as a clam.
To make it worse, I rebooted my Zend box and magically it started working again, but my friend assures me it still fails on his.
I KNOW it doesn’t work on 3.0 multi-site (or WPMU), but that’s because the hooks for email addresses are different.
That’s unfortunate and I hope you’ll eventually find a solution, the only other plugin I found which does something similar to yours is WP-Ban but it seems that the author might not continue to support it.
Gene53 you might consider trying Bad Behaviour. I eventually gave up and disabled Ban Hammer (nice idea though and good to see it works elsewhere). I use Mingle so some options were out but using BB, I’ve probably had 4 spam signups in a week, as opposed to about 160.
I agree with MsCrow. I use Bad Behaviour as well as BH (I was trying to stop ONE guy from signing up on my site).
I’ve been playing Plugin-Compatibility with this in the meantime, so if you’ve got any plugins, let me know. It can’t hurt to keep thumping away at it 🙂
