Erik, probably it’s conflicting with some other plugin, you may need to deactivate your plugins then activate them one by one to figure out the conflicted plugin!
Sadly I have the same problem also try to disenable some plugins!

@hesham: Just a generel qustition to microformats, is it possible (so that google recognizes it) to set the title in the begining of a rewiew and e.g. the rating and description at the end of the review?

The only problem I can think of is a bad function naming!
So I assume that you are using a plugin that is using same function name of one of my plugin’s functions!
Please, try disabling all plugin on your blog, then enable them one by one till you discover the one that crash the site, let me know which plugin so I can check it out.
Answering your question… Yes, Google will still recognize your review no matter where you include the snippets.

I tried it on to different blog (also different servers) but the side always crashed when I activated the plugin! I tried to deactivate plugins step by step till no plugin was active and the site still crashed!
Tried to copy only the author-hreview.php into the plugin folder and activatet it. The site doesn´t crash now, but the plugin is not working and I got the following message:
The plugin couldn´t be activated cause the plugin produces a Fatal Error… (this is not just a translation from me 😉 )
Warning: require_once(include/review_meta_box.php) [function.require-once]: failed to open stream: No such file or directory in /…/wp-content/plugins/author-hreview.php on line 32
