[Plugin: Appointment Calendar] Great Start, but too many errors

This plugin looks great, but really needs some fine tuning.

Some of the issues we found:

1. Did not work when inserted with shortcode into page.
2. Slow to execute when booking appointment
3. Multiple errors on appointment submission

Get the bugs shorted and I think this will be a top plugin..
