 * I need a simple plug in to us on my site as a volunteer form. What i need on 
   the form for example is:
    First Name Mid Int Last Name address address 2 city
   state zip email
 * these are just a few that I need. I will need to be able to add more lines than
   this. I would like for it to be on a page and people can fill in the blanks and
   submit their info with out having to register.
    Anything out there I can use.

 * I have tried this one, but when I hit ‘send’ it doesn’t send all the information
   like what I have listed above.

 * Then I can only suggest that perhaps you didn’t set up the forum correctly. Especially
   the fields that you wanted to be sent by email. Did you read the plugin’s documentation?
