Our organization has WPMU Domain Mapping setup on our domains but for some reason some users are receiving the “This domain has just been registered…” default page. Not every user but some of them. Any ideas on what to check?

That’s a server configuration issue. If your request does not get to a WP generated result then the request isn’t getting to the DM plugin.
