    Simple Contact Info 1.1.3
 * Plugin activated without Warning and any Notice.
    What version WP and plugin 
   did you use?

 * I got a similar error and am using the same latest versions like you (3.6.1 &
   1.1.3)

 * I got the message to go away by by adding an extra input parameter to the update
   function
 * I changed line 172 from
 * `public function update($instance) {`
 * to
 * `public function update($instance, $oldinstance) {`
 * in accordance with the class definition at [http://codex.wordpress.org/Widgets_API](http://codex.wordpress.org/Widgets_API)
