PHP Notice: Undefined index

I’m following this too as my debug.log is FILLED with these same errors (same line number).

One suggestion I found was to delete/reinstall: /wp-content/mu-plugins/gd-system-plugin/plugins/limit-login-attempts-reloaded plugin, but when I try to delete it, I get permission denied.

Another suggested to go into the database to the wp_options table and delete the “limit_login_lockouts” record (though admittedly, I’m hesitant to do that.)

I contacted GoDaddy (hosting company) and they suggested it was as script or plugin issue, but couldn’t offer much more.
