Photos won’t show up on insert – erroenous code added

I just started having this problem today…
When I try to insert a photo into my post (using the visual tab), I’m able to upload my photo and can see it in the gallery. When I click on “insert into post”, it returns me to the post, but nothing has inserted. When I look at the HTML, erroneous code appears in my link to the photo (example: <img class=”alignnone size-medium wp-image-855 dibioyzfldqmcospzegr dibioyzfldqmcospzegr dibioyzfldqmcospzegr dibioyzfldqmcospzegr” title=”WARREN” src=”http://www.lolaludesign.com/blog/wp-content/uploads/2009/10/home4-300×250.jpg” alt=”WARREN” width=”300″ height=”250″ />). If I take out the erroneous code and go back to visual, the photo still doesn’t appear and checking back in the HTML, the code has reappeared. I’ve tried clearing my cach and restarting the dashboard, but I get the same results.
I did upgrade four plug-ins yesterday, but I don’t remember which ones there were. Is there a place to check? I thought if I deactivated the plug-ins that were upgraded, it might fix the problem. I tried to deactivate the ones I think they were, but it didn’t fix my problem.
Can anyone assist me and/or shed some light?
