That’s really strange, particularly the part about Offline Donations.

I think this might be related to the jQuery error I see on your page. It’s related to the flexslider. That’s perhaps in your theme or a plugin. Is there a way for you to disable that? Try that and see if that resolves the issue.

I changed to template since I couldn’t figure out where the flexslider was being call. This is the error I get when I try to submit an offline donation.

    Uncaught TypeError: Cannot read property ‘checkValidity’ of undefined(anonymous function) @ give.all.min.js?ver=1.3.0.3:8m.event.dispatch @ jquery.js?ver=1.11.3:4m.event.add.r.handle @ jquery.js?ver=1.11.3:4

The flexslider error is still present on the page as far as I can see right now. Here’s what I’d suggest:

    1) Disable ALL other plugins. If the problem is gone, then start re-activating each plugin one by one until the problem happens again. Let us know which plugin did it, and we’ll try to duplicate the problem.

    2) If after disabling all plugin the problem is STILL there, change your theme to TwentyFifteen. If the problem is gone, then we know it’s your theme. If so, email your theme to us at support [at] givewp.com and we’ll see what we come up with.

Let me know how that goes.
