The language from the smart buttons and PayPal interface is automatically decided by PayPal on a per-user basis. The language detection does not work as well on iOS devices as it does on any other mobile devices though, so it can potentially fall back to English as a default.
To change the language manually, there is usually a language switcher at the bottom of the page though:

But we are working to integrate a feature that allows you to override the PayPal interface language with your WordPress locale so that your users would get to see a German PayPal interface regardless of other factors like a different system language.

look in my loom: The switcher isn’t even showing german. This is a big problem. When will this be fixed? We cannot use this plugin in this state..

The content of what you see in this window depends on several different factors which are out of the scope of the plugin for now, as PayPal automatically determines the most fitting interface and its language. Most of the time it works well, but we have informed them about the issue with iOS devices.
The next feature update will likely arrive sometime in January, but I can’t say too much about it yet as we are currently focused on stability improvements.
There will be several enhancements for the German market in particular around Q1/Q2 when PayPal Payments officially replaces the current PayPal Plus plugin for the German market.
