[PATCH] Warnings and bug fixes

In WP 3.6.1 with WP_DEBUG turned on your module puts out a few warnings (undefined indexes, deprecation warnings etc). It also has a couple of bugs such as not closing comments correctly on locked pages. There was a bit too paste here so I made a git repo. Check the commit log for full details on fixes.
