Pages Permalink Structure

I use a custom permalink structure to help keep bog my as search engine friendly as possible. The one think I do not link is how the page permalink structure works.
Here is an example of how it looks now…
http://www.domain.com/page-name
and
http://www.domain.com/parent-page/page-nameHere is an example of how it should look…
http://www.domain.com/page-name.html
and
http://www.domain.com/parent-page/ (parent page index)
and
http://www.domain.com/parent-page/page-name.htmlIt would be fantastic if this could be implemented into an upcoming release of WordPress. In the meantime does anyone know how a hack that does what I listed above?
