Are they showing a 404..?

Are the missing pages still listed in your menu in Appearance > Menus? If not, add them back.
They are all listed under Menu Structure in the correct order, but only 3 of them are showing up on the site.
No 404 – the links are just gone.
I only see the three items in the menu there. Try going to Appearance->Menus and modifying that menu. Add something, remove something, etc. See if that has any effect.
I tried that. And then I tried scrapping the old menu and starting from scratch and still only the same 3 menu options are showing up.
I upgraded to 4.0 this afternoon and a handful of my nav menu pages aren’t showing.
Please help.
I’ve had a similar issue. Since updating, my normal menu has disappeared and it is instead showing my blog menu. Had a look in menus and it’s still there.

I managed to get mine fixed. I don’t know if you have the same folder structure, but in my templates folder, my header-default.php was missing:
‘theme_location’ => ‘primary_navigation’,
