    My Site http://www.acumen-services.com was recently hacked, huge code was injected on this.

    After so many efforts, after read many articles, after lots of research and after a lot of help of professional experienced people. Injected code removed. But its still having urls ?page_id=52, ?page_id=54, ?page_id=50 like this. I am unable to understand where they injected this code. its only showing when permalink comes on default mode. Caching in search engines too.

    Please help to make it clean.

    You need to change your permalinks setting. From dashboard Settings > Permalinks, make your choice and SAVE. Then test that they are working properly.
    Once they are working properly, then (assuming that you are using Google webmaster tools), you can go to the management page in google webmaster tools and request your site be rescanned, this will speedup clearing the cached entries.

    Still urls are generating ?page_id=52, ?page_id=54, ?page_id=50 like this.
    Can you please tell me from where i can remove these on website.

    I have removed huge injected code from many pages. After that make disallow these urls in robot file, so that google ignore these urls. But actually problem is where these urls are injected.

    I have removed htaccess file also. Permalink setting is also done.

    Your site is still hacked.
    Your priority is to secure your site and remove the bogus code and content. There is no point in doing anything else until you have done this.
