Page not found

Hello

I have just installed latest version of WordPress 4.6.1 and Your plugin version 0.19.4.
I removed other plugins.
I am using default template.
“WordPress 4.6.1 running Twenty Sixteen theme.”

I created simple test with 1 question.

After clicking “Get Test Results” I get error “404 Page Not found”

Here is my PHP info site.

I asked my hosting support to check if there are any errors, but they didn’t find anything.
I don’t see any errors in my Apache error logs.

Could You help me ?
