The documentation at the top of your files is wrong.

* You can change the widget output by copying any of the three template files
* in the /fpw_views-2/ folder to an /fpw_views-2/ folder in the active theme.

Its not fpw_views-2. Its fpw2_views

@graftedin, thanks for catching this. I found four instances of that mistake and have fixed them all. The fix will go out in the next update.
