Options for comment disappear

Hello,
I have a problem, on my site all the options do not appear… for example I have no fields with name email ect… the users can only sign with Facebook, hotmail etc… when I refresh the hanging page the options display 1sec then disappear… if I cross myself with Facebook and what I made a comment this one does not display in the guestbook either…can u help me please?
