    I am getting this error which I suspect is not actually due to this particular plugin but a wider issue but it does appear most often for FormMaker.
    file_exists(): open_basedir restriction in effect. File(C:\Inetpub\vhosts\xxxx\httpdocs/wp-content/plugins/https://wordpress.org/support/plugin/form-maker/#new-post) is not within the allowed path(s): (C:/Inetpub/vhosts/xxxx\;C:\Windows\Temp\;”C:\Inetpub\vhosts\xxxx\httpdocs”) in C:\Inetpub\vhosts\xxxx\httpdocs\wp-admin\menu-header.php on line 256

    Can you provide any clue as to what in the plugin is making a call to menu-header line 256 so I might see what is causing it?

    Further research on this (which is probably beyond my skills!) suggests that for some plugins the data in menu-header.php
    function _wp_menu_output( $menu, $submenu, $submenu_as_parent = true ) {
    global $self, $parent_file, $submenu_file, $plugin_page, $typenow;
    has been corrupted in that the the $submenu array sometimes contains a URL instead of the menu item data
    Does this sound possible? Where does this data come from? And how might I fix this?
