# “Oops…Something Went Wrong” Message

 * Hi there! I setup an opt inform but it does not work. Here’s the error message
   I’m getting:

 * The other forms on our website work. I’m assuming I’ve made an error setting 
   up this one. Any help would be appreciated.

   This means that the fields FNAME and LNAME are set as“
   required” but not added to the form. Either set those fields to not be required
   under your Audience settings > Fields, or add them to the form. Hope that helps.
