Off the shelf php code to interrogate (and return) a MySQL db

Hi – I’m looking for two things.

    1. Off the shelf, ready to tweak php code, that allows a visitor to choose their own search criteria to search my MySQL db and then report the results, and

    2. Advice of where to put the php code, and how to set up the HTML to collect the user choices and run the script.

    Thanks,
