Not receiving form info

This form: http://www.dynastypoolsri.com/info-request/ I am not receiving info there. I have it going to my domain account @dynastypoolsri.com and cc’ing to my gmail account. Only the gmail account is receiving. I have tried everything and am at a loss. Any suggestions?
