The page sends me to 404 page, please check.
And does it start happen all on a sudden? I can see you have other threads, so was it working before?

I’m SURE it was working yesterday – but I was also playing around with Email from Download (but it was waaayyy to complicated – your plugin is much more simple and user friendly!) yesterday too so I might be getting confused! But I’m sure it did work at some point yesterday.
I’ve now deleted the Email from Download plugin incase that was interfering with this plugin

Does normal email work for you now? I mean, try recovering password and check if you get any email. Please check spam too.
Yes, all my emails work fine – and I’m able to send/receive.
I THINK I might have some more relevant information! I checked back through my emails to see what I’d tested yesterday and found the email – however the download link was wrong, this is what it said:

So I changed the the link to the WordPress URL download link? And it was from then that the form stopped sending out the email!
Could this be the issue? and if so, how can I fix the broken link? 🙂

I changed the download link back to the original:
Download here: {{download_link}}
But it’s still not sending any email through – I’ve run out of email addresses to test now and am unable to delete the existing data, even after deleting the plugin and re downloading it – the data still stays within the plugin?
