I am using the plug-in on a couple of sites that have WP 4.1.5 and it is working. Installed it on a site that uses WP 4.2.2 and all the source code is there, but the icon does not display.

Plugin conflict. You need to test out by eliminating other possible conflicts. If 4..1. or 4.2 did not work, then you would see others posting.
