    Hello, normally if the images or sizes said deleted, the image is deleted from the server and the database. But i build this plugin with one logic to prevent any deletion problem, so if at the server the image stay the plugin will not delete the information in the DB.
    You can send me somes snapshot before and after the delete action?

    I was working to add one log/debug/error system, but at this moment i can’t finish him, but i will the next or two week.

    You can see in the console with the ctrl+shift+k or ctrl+shift+i if you get any error or see what happen went you try to delete one image, like this http://www.nicearma.com/wp-content/uploads/2016/01/debug_dnui.png (is in french, but all option are in the same place, this is firefox, and reseau is network, and apercu is preview)

    I was thinking if the issue has something to do with the grotesque amount of files I have in one single media folder. Ftp maxed out at 25.000 files in one folder but theres at least 4 times as many files attached to that folder.

    No, the amount of image is not a problem, i really don’t know what is going up, normally the image is deleted, witch version of the plugin you use? Is the 2.2.2? Normally the version 2.2.1 had a similar bug, you can find some one with this problem https://wordpress.org/support/topic/how-to-not-delete-original-images?replies=34 but normally the version 2.2.2 fix that. The problem was because the upload folder was not created, but the backup options was active, so maybe try to create the backup folder and see if work fine.

    And be careful, it seem that you have size like widget, this can be use out of the main logic of WordPress, so verify by you own if the not used label is really not used (do not go crazy deleting all size without some verification )

    I think I found the error. I hadn’t created the backupfolder. I pushed “Create” backup-folder and it looks like it works.

    The hosting provider is going to create a back up zip-file. I’ll wait for that before I take the axxe to my unused media.

    Normally i fix this plugin, now if the folder doesn’t exist, the default value of backup will be false
