Not connecting to the Thank You page?

Hi, I’m loving the possibilities with this plugin but having trouble implementing it. I’ve added the shortcode as part of the template of a long single landing page, but upon entering the information, there’s no redirection to the Thank You page.
Page: http://wordpress-rxactive.rhcloud.com/ (“Sign up” button)
Submitting the form will do something, as the URL reads (for a blank entry): “http://wordpress-rxactive.rhcloud.com/?name=&email=&signup=Sign+up”, but there’s no redirection to any of the Thank You pages. (Any chance it’s a conflict from jQuery versions…?)
Would really appreciate any help/guidance!! Thank you!
