No solutions work! Cannot login after 4.0 upgrade

Not sure if I’m current at version 3.8 but attempted to upgrade to WP 4.0. When I go to login, the blank boxes to login appear again and the url changes to “http://unitsmilwaukee.com/wp-login.php?redirect_to=http%3A%2F%2Funitsmilwaukee.com%2Fwp-admin%2F&reauth=1”

I’ve researched and attempted these fixes without any luck:

Re-uploaded via sftp the wp-settings.php and the session.php files via a fresh download of 4.0

I’ve re-uploaded all folders and files except for the wp-config.php file.

The strange thing compared to other’s issues with this is that I do not get an error message.
