 * There is no proper instructions to design all pages if you creating your own 
   template. There is a option to assign template for first page but no option for
   the page after filling the form.

 * You can change any element via CSS classes that have been assigned to every element
   of the plugin

 * I am editing twentytwelve theme for a website. I am creating custom page templates
   for pages. Now there is a option to assign a template from pages section to the
   first page on which form is coming but there is no option to assign a template
   to the page which come after filling the form.

    I’m not sure how aware you are of how damaging this one star review
   is for our plugin – especially considering it was more of a support issue.
 * We have provided a solution to the issue you experienced. This is the only way
   that you can modify the template for a “custom post type” in WordPress. We have
   also provided an example template file for you to use on the link specified above.

   suits your needs.
