Maybe during your tests, Google picked up an older cached version of the page.

The image alt and title attributes on the product page seem to have an index number added to the end (2, 3, 4, etc.)

Additionally, all image attributes follow the exact same format, apart from the last number.
So this is why I think that Woo Image SEO is working correctly and applying the image attributes to the product in question.
