 * the add image and media buttons have disappeared from our dashboard all of sudden
   and now we cant add images to our posts..help!!

 * Same thing with me. In my case, they were in place afer switch to 2.7.1 and gone
   after I updated jQuery.js file. Cleared cache, cleared cookies — not working.
   The strange thing though — in full screen mode there are these buttons, but they
   are not working. In dashboard the buttons are in place and working, but not above
   post form.

 * I dont have any ‘Media’ Button on my Dashboard. It shows only Dashboard, My blogs
   and Profile button.. Please help!

 * seems that some files are missig. Reupload wp-admin and wp-includes folders.
 * Also, deactivate all your plugins
