New Contact form fails to send and directs to paypal

I am using Contact7 successfully for one form.

However, I tried to make a second form (for a different purpose) and it is not working at all. When I hit Send, it fails to send the form, and I am directed to paypal.

I have tried various things including:
Copying and editing a Contact form that is already working (and it is still working)
Editing and even removing the additional instructions to redirect after Send.

I am at a loss and would appreciate any advice.

Is it possible that a user is only allowed to use one contact form at once?
