Need to add "Upload File" button to form…help?

    Hi:
    I am now thinking I will need to find a developer or freelancer to help me with this. I want a secure (captcha?) form that will allow my clients to send me info (like to print a business card, etc.) and upload a file that will all be sent easily and quickly to me (maybe an email with a link to the file on my server?).

    I am building a site now (not live yet). And cannot find any extension to help me with this…

    Any suggestions or help is appreciated!
