Need Job Application Plug-in – Conditional Form?

Hey Folks. I’ve got a client looking to put a job application online. The have some requirements that are conditional. I’d want the form to change if based on the specific job they are applying for and what-not. Hopefully this information could be fed into either a spreadsheet or populate another document for easy review. I’m willing to spend some money to get a good looking form.

Any Ideas?
