Need a Plugin for Form entry and exporting

    Hi there,

    I was hoping if anyone could direct me to a plugin(s) that allows users of my site to enter information into a form which is then reproduced into a page on wordpress for other visitors to see.

    Something like this:

    If you need better visualisation, imagine a website like zoopla.
