    Can you please provide a screenshot of what you see? The form is showing fine for me.

    It’s showing now because I fixed the problem by reverting to a back up of my site. I don’t know if it will return if I upgrade. Your response was extremely so and I had to do something urgently.

    Ok, if you want to create a staging site and update to latest version so issue shows I can have a look.
