My form is not showing on my website

    Hi I am new to WordPress so be kind to me. I downloaded the free plug in I just wanted to add check box options to one of my products I created the form but its not showing on the product I would appreciate any help you can give me.

    Hi,
    It seems there is some php errors in your site.
    The content is broken after the product image and price.
    Please enable debug in wordpress (https://wordpress.org/support/article/debugging-in-wordpress/) so that it can view the actual error causing it

    It seems you are using very old version of woocommerce, You need to update your woocommerce in order to work our plugin
