    I have had multisite running on my domain for quite a while. After performing an upgrade, all of my sites are the same name, and map to the same URL/admin/etc… When I add a new site, it just add’s another of the same. Any idea what has happened here?

    Since there’s been no recent update of WP, I’m going to go out on a limb and guess you were VERY out of date. If so, did you remember to update all your plugins and themes?

    This issue happened with W3TC about a year afgo.

    Nah, when this happened, it was only a dot release or two. Nothing too major. I am only posting about it now because there were workarounds to get to administer the other sites. But now that I need to add a new one, I am unable to. I just made sure all plugins and themes were up to date, and am still having the same issue. Is there any sort of cache to clear, or maybe it has something to do with permissions and/or htaccess?

    Are you using any caching plugins?

    Ah ha! That did the trick. Deleted the caching plugin and it fixed it. Thanks!
