 * Thanks for your contribution to the plugins. Surveys are one of those areas that
   has not really been nailed properly by any single plugin without premium costs
   or cloud services getting in the way!
 * I have tried the plugin on WP multisite and it appears to work at the admin end
   but on publishing, no fields are available, radio and pulldown formats dont show.
 * This is with no other plugins and a vanilla Twenty Fourteen theme.
 * Do you have any ideas or plans to help fix it?
