Multiple options on one form

I’m using newest version of WP 4.7 and newest GF 2.1.1.12. I’m an experienced GF user but with MailChimp. I have a client using CC and I’m struggling to figure this out. I’ve setup the feed and mapped the fields. I have tested to see if it works and it does for the main list. But what I was hoping to do was have a single form and let the visitor choose 1, 2, 3 or all 4 lists to join on that single form. I don’t see how that is possible. When I create the contact form I don’t see any other way except one form to one list. If I’m correct, I must create a separate form for each list, is that correct?

Am I just missing it? I’m worried that there’s so little activity on this forum that the plugin may no longer be viable…and it doesn’t appear there are other GF/CC options. I actually emailed GF for help and they said this was the only option and I had to get help here not from them. THANKS!
