Forms on our testing installation appear to preview accurately.

Would you be able to send an export of the form that you are experiencing difficulty with (instructions for export can be found at https://docs.gravityforms.com/exporting-a-form-from-gravity-forms/)? It is also useful to have a Gravity Forms -> System Status report (https://docs.gravityforms.com/checking-environment-details/). Please could you send this information through to hello@webholism.com for further investigation?
