# Multiple Booking Form Fields

 * We are hosting a volleyball registration form on our website. The teams will 
   consist of four people so we will need to have multiple booking form fields including
   first name, last name and email address. After creating all of the fields, the
   form automatically condensed the multiple fields into one. Is there a workaround
   for this particular situation?

 * Hi Angelo,
 * Perfect! We have purchased the Pro add-on, but I don’t see a place to Enable 
   Attendee Forms in Settings.
