By default “multiple” choices will be rendered via “comma”. Unfortunately, we are not sure what you mean by “giving each unique response”. Do you need to put “bad” in one column, “good” in 2nd column, and “very good” in 3rd column? Is it the column “HTML” or its separated objects? Or it is a “checkbox”?
Maybe you can attach a screenshot of your E2Pdf Template with marks on where and how you expect the output.

Is it what you are trying to complete? On the above screenshots:
1st element will be shown if [123] will contain “bad” in value, for example: bad, good, very good, if the checkboxes will be good, very good – it will not be shown.
2nd element will be shown if [123] will contain “good” in value, for example: bad, good, very good
P.S. In your case, it could be needed to use “separate” values and the shortcode as [123 show=”value”]
If it’s still not what you are looking for or it’s not working as expected, create please “textarea” and map it with shortcode as [123] and [123 show=”value”]. Attach the output of the “textarea”.
[123] must be changed to your “checkbox” Field ID.
