Mobile views not displaying properly

My team has created this form for our site and it works great, but for some reason when view on mobile, the text boxes do not behave responsively and you have to scroll horizontally to view the rest of the contents.

Can you please tell me what to do in order to fix this and allow for a cleaner presentation on mobile?

Also, we have noticed on iPhones that the font is displaying as Times New Roman and not the default font for our site (only when viewed on Mobile, and I believe only with iPhones).
