This must have been added by a plugin. This seems to be no longer active in your project. I guess it is about a form plugin. Have you changed anything in this regard?
hello thank you for your reply. No it’s not a plug-in. I haven’t added any new plug-ins for a about 6 months. I used it as a contact form for customers. It worked very well and has just disappeared. I would very much like to get it back and working.
i have recently updated the WooCommerce database. That is the only thing I think it could be connected to.

Such a function can also be supplemented by themes. Have you ever changed this, also by an update if necessary?
What did you see under the feedback menu item?
WooCommerce has no feedback menu integrated to my knowledge.
i use a child theme based on storefront. But again I haven’t updated that either.

were you possible using JETPACK plugin? that might be related to the “feedback” and “contact form”

Found a debug link on jetpack settings page which had a link for all the options within jetpack. The one for contact form was showing as inactive. After clicking this to activate the problem was solved.
