    WP v3.2.1.

The "Add-New" Tab under Appearance and "Add New" under Plugins, disappeared.
    How to fix this?

    Try:
    - switching to the Twenty Eleven theme by renaming your current theme's folder in wp-content/themes and adding "-old" to the end of the folder name using FTP or whatever file management application your host provides.

    - resetting the plugins folder by FTP or phpMyAdmin.

    i found the solution:

after rename of the table prefix i found in the _options table two entries with "_user_roles"

one with regular prefix "wp_user_roles" and the one with the new prefix like that "xy01_user_roles"

i deactivated the old "wp_user_roles" and all works fine
