Membership form distorted, please HELP!

    All of a sudden our WP-members form has become distorted, the first name field doesn’t appear normally. Eliminating the first name field only causes the next field down to distort. I have deactivated all other plug ins one by one to see if that resolves the issue, unfortunately it didn’t, deactivated/reactivated the WP-members plug in several times too. Tried creating a new page putting the short code on the new page, unfortunately the form is still distorted on the new page. We’re running WP ver. 3.9.2 and WP-members ver. 2.9.5. The WP-members plug in has been working flawlessly for over a year, just became aware a couple of days ago that it’s experiencing problems, below is a link to our membership form.

    There are several stylesheets included with the plugin. You can also load a custom stylesheet. I’m not sure if you changed what you have set because what you have there now looks fine in my browser.

    If you have trouble and lack the skills to create a custom stylesheet, I would recommend using one of the “no float” stylesheets included with the plugin. These don’t float the elements and work with a wider array of themes.
