My client would like to have a forum and he would like to know if when a member signs up we can ask the member for Name, Email, City, State, and Organization.

I’m sorry but wpForo doesn’t have user field customization feature. You can extend and add field managing options using wpForo – Users Custom Fields paid addon: https://gvectors.com/product/wpforo-user-custom-fields/
