I’m having exactly the same problem, but only when I change the Language in general settings to Portuguese.
I need that because of a translator application I am using.

Seems we are three of Us, suddenly I can’t add images to pages in a website with WordPress 4.2.4. I have CK Editor installed so from the image icon there I’m pulling them from library with the image URL
Were you guys able to troubleshoot this issue?
I deactivated all the Plugins and that didn’t do anything. I started having this issue when I downloaded the new WP update (v. 4.2.4). So, I’m thinking the update may have caused the issue.
I’ll attempt to revert to the previous version of WP and see if that works. I’ll keep you updated.
Diagnosed the issue.
First of all, check to see if Plugins are causing the issue by deactivating all the Plugins, checking if this fixes the issue. If it does, then one, or more, of the Plugins might be causing this issue.
If that doesn’t change anything, then check to see if your error lies in your theme. You can check this by switching you WP theme to one of the defaults. Then, check if the Media Library works. If it does, then your error is probably a bug in one of your customized theme files.
The second is where my issue lied. I had a bug in my functions.php file. I figured this out by deleting everything in that file and checking if the Media Library worked. It did. Then I added functions to the file one-by-one and sure enough I found a bug.
